Job Description

Overview Kidney Pancreas Surgery – Full Time Under general direction and in collaboration with the multidisciplinary transplant team, the nurse coordinator provides expert nursing care for transplant patients, including candidates (pre-transplant), recipients (post-transplant) and/or living donors with a variety of complex treatment options. The nurse coordinator serves as a contact point for transplant patients, families, referring physicians, and staff, and coordinates and facilitates care in specialty areas. The nurse coordinator provides leadership for collaborative and multidisciplinary patient care with emphasis on education and continuity of care for inpatient and/or outpatient settings. The transplant nurse coordinator monitors and coordinates care to help patients reach their maximum health potential before and/or after transplant. Responsibilities Triaging patient calls and lab results Coordinating studies and specialty referrals Confirming orders and providing ongoing transplant patient education Spearheading coordination of care across disciplines; clinic work as needed Pre-transplant coordinators focus on evaluation/workup of the pre-transplant candidate Post-transplant coordinators focus on coordinating care and optimizing health of the transplant recipient Living donor coordinators focus on evaluation/workup of potential living donors Communicating with patients on behalf of providers and responding to changes in patient condition Adjusting medications and treatment plans as delegated by the provider; educating patients on changes and documenting in the EHR Location, Schedule & Compensation Department: Post-Kidney Transplant Program Location: UCSF Health, San Francisco, CA (Parnassus Heights) Work Style: Fully On-Site Shift: Days; 8 Hours; Monday–Friday Salary range: $95.76 - $124.25 per hour; final salary determined by experience and internal equity Required Qualifications Minimum one (1) year of outpatient RN employment in organ post-transplantation or related organ specialty Knowledge of end-stage organ-specific disease, transplantation, and immunosuppressive therapy Understanding of transplant contraindications and common post-transplant complications Proven organizational skills and ability to assume responsibility for a defined area Ability to work effectively with physicians and support staff Excellent written and verbal communication; professional conduct Computer literacy (MS Word, Outlook, internet) and willingness to learn EHRs Ability to meet deadlines, work independently, and prioritize workload Clinical leadership, teaching, and communication skills or equivalent experience Flexibility to orient and work at all UCSF Health locations Preferred Qualifications Minimum three (3) years of relevant transplant experience Prior outpatient transplant experience Certified Clinic Transplant Nurse (CCTN) certification Licensure & Certification Active Registered Nurse licensure in the State of California Active American Heart Association CPR Certification About UCSF At UCSF Health, our mission is innovative patient care, advanced technology and pioneering research.
